About the role
Join Country Health Connect as a Clinical Nurse – Continence Advisor, providing specialist nursing services to clients in the community in the areas of continence and stomal therapy.
In this predominantly clinical role, you will provide proficient, person-centred nursing care and individual case management, assessing client needs, implementing appropriate therapeutic interventions and monitoring outcomes. You will coordinate services across multidisciplinary teams while supporting evidence-based practice, quality improvement and continuity of care.
You will also provide clinical leadership, education and guidance to students and less experienced nursing staff, while contributing your specialist knowledge and experience to service improvement, clinical projects and professional practice across Community Nursing.
You are an experienced Registered Nurse with demonstrated competence in continence nursing practice and a commitment to delivering high-quality, person-centred care in the community. You bring strong communication and clinical decision-making skills, work effectively within multidisciplinary teams and are confident providing leadership, education and support to less experienced staff.

Country Health Connect Community Nursing provides community-based nursing services across the Limestone Coast. As part of the Community Nursing team, you will work collaboratively with nursing and multidisciplinary health professionals, as well as government and non-government organisations, to support the needs of clients across the region.
