We are seeking multiple passionate and client-focused professionals to support individuals and families who are experiencing homelessness or are at risk of entering the homelessness system.
As our Divert Case Manager, you will play a vital role in helping people navigate housing crises, access emergency accommodation, and connect with services that support long-term housing stability and wellbeing. Working from a person-centred, strengths-based and trauma-informed approach, you will support clients to develop individual plans and achieve sustainable outcomes.
Working closely with the Toward Home Alliance, Lutheran Care teams and external stakeholders, you will contribute to a collaborative, integrated service model focused on reducing homelessness and ensuring people receive the right support at the right time.
