Coordinate access support for Aboriginal and Torres Strait Islander people seeking assistance with accessing the National Disability Insurance Scheme (NDIS). Work collaboratively with ATSICHS Brisbane health professionals, allied health practitioners, and external service providers to support clients through NDIS Access Requests, evidence gathering, reviews, and appeals. Assist clients to understand the NDIS and access appropriate supports while promoting culturally safe and person-centred practices.

Duties

• Support prospective clients through the NDIS Access Request process.
• Assist clients to understand NDIS eligibility requirements, processes, and available pathways.
• Coordinate with ATSICHS Brisbane health professionals, allied health practitioners, and external service providers to obtain documentation and supporting evidence required for NDIS applications.
• Work collaboratively with General Practitioners, Occupational Therapists, Psychologists, Speech Pathologists, Social Workers, and other practitioners to support the development of evidence demonstrating functional impairment and support needs.
• Facilitate the completion and submission of NDIS Access Requests and related documentation.
• Support clients through NDIS reviews, reassessments, and appeals processes where required.
• Provide information regarding the NDIS and available disability supports to clients, families, carers, and community members.
• Maintain accurate, timely, and detailed case notes, participant records, and file documentation in accordance with organisational requirements.
• Monitor application progress and maintain regular communication with clients throughout the NDIS access process.
• Liaise with the Carers QLD and the National Disability Insurance Agency (NDIA) and relevant stakeholders regarding application outcomes and information requests.
• Refer individuals who are not eligible for the NDIS to appropriate internal or external supports and services.
• Report on program activities, client outcomes, and key performance measures.
• Assist in the planning and delivery of NDIS information and community awareness sessions.
• Contribute to the development of resources and information materials relating to NDIS access and eligibility.
• Maintain quality assurance standards and ensure compliance with organisational policies and procedures.
• Stay informed of NDIS legislation, operational guidelines, and sector changes relevant to access and eligibility pathways.
• Travel between work sites on a regular basis.
• Travel to work in the community on a regular basis.

Benefits

• Starting salary of $78,272 per annum (depending on qualifications and experience).
• Additional 12% Super + Not-for-Profit Salary Packaging (Increases your take home pay).
• 12-month contract (Full-time or Part-time, minimum 4 days).

COVID - 19 Requirements

• Immunisation history showing you have received two doses of a Covid 19 vaccine is encouraged.

Required

• Certificate III or higher qualification in Health Care, Social Assistance, Community Services, Administration, or a related field.
• Knowledge, understanding, and/or awareness of the disability sector, service access pathways, and challenges participants may experience in accessing supports.
• Demonstrated ability to conduct intake, screening, assessment reviews, referral, and service navigation activities.
• Demonstrated ability to record accurate, objective, and consistent case notes and client records.
• Demonstrated ability to apply professional judgement when supporting clients to identify goals, build capacity, and access appropriate services.
• Demonstrated ability to work with minimal direction and effectively communicate and collaborate within a multidisciplinary team environment.
• Strong organisational skills, time management, ability to work individually and as a team, and ability to manage competing priorities.
• Demonstrated ability to communicate sensitively and effectively with Aboriginal and Torres Strait Islander people, families, communities, and stakeholders, using culturally safe and person-centred approaches.
• Demonstrated ability to advocate for clients and support them to navigate complex health, disability, community, and social service systems.
• Current C Class Driver Licence and ability to undertake regular travel throughout the community.
• Knowledge of the health and community services sector, including medical terminology, common health conditions, disabilities, mental health presentations, and the ability to interpret and communicate health-related information effectively with clients, carers, and healthcare professionals.

• National Police Certificate.
• Current Working with Children's Card (Blue Card) or eligible to apply.
• Current Disability Worker Screening Worker (Yellow Card) or eligible to apply.
• Current Driver's Licence.
• Immunisations Category B.
• Evidence of Work Rights.
