About the Role
Join CO-ACT at Hampstead Rehabilitation Centre and help older people achieve safe, meaningful transitions from hospital to community care.
• Assess and deliver tailored physiotherapy for complex patient needs and support complex discharge planning
• Manage a varied caseload with growing clinical independence
• Partner with multidisciplinary teams to support quality patient care
• Educate colleagues and provide clinical guidance to less experienced staff
• Contribute to quality improvement, research and community care planning
About You
We’re looking for a skilled physiotherapist who brings sound judgement, teamwork and a commitment to quality patient care.
• Physiotherapy degree eligible for AHPRA registration
• Experience in acute or rehabilitation settings with adults
• Sound clinical judgement managing complex caseloads independently
• Strong communication, teamwork and evidence based practice skills
• Commitment to professional development, service quality and safety
