Grade 2 Social Worker

About the Role

This Grade 2 Social Worker role is an opportunity to join the Eye and Ear Social Work team and contribute to the delivery of high-quality, patient-centred care across inpatient and outpatient services.

Working closely with multidisciplinary teams, the role provides a range of social work services including psychosocial assessment, counselling, advocacy, discharge planning and referral to appropriate supports and services.

The position also contributes to key hospital initiatives, including the Strengthening Hospital Response to Family Violence (SHRFV) initiative and implementation of the Multi-Agency Risk Assessment and Management (MARAM) Framework. This includes supporting staff, contributing to risk assessment and management, and facilitating appropriate referrals and responses.

The successful applicant will work collaboratively with patients, families, hospital teams and external services to support positive patient outcomes and wellbeing.
