Community Care Foundation is looking for an experienced Participant Services Facilitator to join our Participant Services team and help lead the delivery of high-quality, person-centred supports across the Fraser Coast and Wide Bay.
This is a hands-on coordination role for someone who understands that great disability services are about more than rosters and paperwork — they are about people, relationships, quality and outcomes.Working closely with participants, their stakeholders and our Lifestyle Support Workers, you will help ensure our services are safe, compliant, well-coordinated and focused on achieving meaningful outcomes for the people we support.

The RoleAs a Participant Services Facilitator, you will play an important role in the day-to-day coordination and quality oversight of allocated participant services and SIL operations.Your responsibilities will include:Coordinating the effective and efficient delivery of participant services and SIL operationsWorking collaboratively with participants and stakeholders to support NDIS goals and positive outcomesBuilding and maintaining strong relationships with participants, families, stakeholders and support teamsCommunicating proactively, professionally and in a timely manner with participants, stakeholders and team members, recognising that timely communication is essential to quality service deliveryProviding day-to-day guidance, supervision and support to Lifestyle Support Workers and Senior Support WorkersSupporting staff training, development, mandatory training and competency requirementsWorking alongside Rostering to maintain appropriate staffing levels and continuity of supportProactively monitoring and managing overtimeReviewing participant documentation, support plans, health passports, risk assessments and allied health plansConducting audits and quality checks to ensure documentation and service delivery meet required standardsSupporting incident investigations and ensuring reports are completed within required timeframesParticipating in recruitment, onboarding and offboarding of participant services and staffSupporting performance and conflict management in partnership with HR and the Team LeaderFacilitating and attending team and stakeholder meetingsApproving staff timesheets within required payroll timeframesSupporting continuous improvement across participant servicesParticipating in a 7-day rotating after-hours on-call rosterProviding direct participant support when requiredThe position description identifies the role as having responsibility for effective participant service and SIL operations, staff support, compliance, quality, stakeholder relationships and continuous improvement.

Essential RequirementsRelevant experience in the disability, community services or similar sectorUnderstanding of NDIS requirements and person-centred practiceAbility to obtain and maintain an NDIS Worker Screening CheckAbility to obtain and maintain a Blue CardCurrent QLD Driver's LicenceAbility to participate in a 7-day rotating after-hours on-call rosterRelevant qualifications in Community Services, Disability, Leadership/Management or a related field will be highly regarded
Highly RegardedExperience in a coordination, senior support, team leader or supervisory roleExperience within SIL or other NDIS-funded servicesExperience supervising and supporting frontline staffKnowledge of participant documentation, audits, incident management and complianceExperience with rostering and workforce coordinationExperience managing overtime and staffing requirementsStrong understanding of Positive Behaviour Support and restrictive practicesExperience working collaboratively with allied health professionals and other stakeholders
