Provide person-centred support to participants across SIL, in-home, community access, respite and social supports.
Assist with daily living, personal care, meal preparation and household tasks where required.
Support participants to access the community, social activities, appointments and recreational activities.
Encourage independence, choice, dignity and participation.
Build positive and respectful relationships with participants and their support networks.
Complete required documentation and follow support plans, safety procedures and workplace policies
Follow individual support plans and organisational procedures.
Complete progress notes and required documentation.
Work collaboratively with families, coordinators and the wider support team.
