CRC is excited to be recruiting for our team of dedicated professionals working to help change women’s lives, and we’d love to hear from you!
As a Transition Worker in our Inner-City Women’s Transitional Service, you’ll be at the heart of a high-impact team. Supported by a program manager and teammates, you’ll use holistic case management to provide specialist case management support for women who are impacted by the criminal justice system and homelessness. Using a throughcare model you will provide a continuum of support to address the complex and diverse needs of women who have experienced involvement with the criminal justice system. Your role will include outreach support both pre and post release, including practical and psychosocial supports, creating community connections and participate in supporting other relevant CRC projects.
Your usual work hours will be Monday to Friday, with occasional out-of-hours work, as required.
