We are seeking a solid, reliable, and Culturally competent Senior Case Manager to lead and support Junior Case Managers and Cultural Mentors while standing firm alongside Participants as they navigate reintegration, overcome challenges, and take ownership of their path back into the community.

Our Reconnection, Employment and Learning (REAL) Program delivers direct, on-the-ground support to Aboriginal people who are in custody or transitioning from incarceration back into community. We focus on practical execution, long-term stability, and strong Cultural standing to ensure participants return home with confidence and dignity.

As a Senior Case Manager, you will play a pivotal role supporting participants through pre-release, on-release, and post-release stages. You will help participants reconnect with community, culture, and essential services, empowering them to build sustainable pathways toward employment, education, and long-term independence.
In this role, you will lead with empathy, collaborate closely with Elders and families, and mentor junior case workers and cultural mentors to foster a supportive, community-led environment.
Key ResponsibilitiesHolistic Case Management: Deliver high-quality, strengths-based support and maintain tailored Individual Action Plans (IAPs) for each participant.
Service Navigation: Coordinate referrals to health, housing, legal, education, and employment services while supporting participants with parole compliance.
Cultural Connection: Collaborate with Elders, cultural groups, and families to support participant reconnection.
Relationship Building: Foster trusting relationships with participants, correctional facility staff, and community organisations.
Team Leadership & Mentorship: Lead and support junior case managers and cultural mentors while contributing to a positive, collaborative team culture.
Cultural Competency: Deep understanding of First Nations cultures, community dynamics, and the systemic barriers faced by participants.
Industry Experience: Proven background in case management or mentoring within justice, community, or employment services.
Relevant Qualifications: Degree or diploma in Social Work, Community Services, Justice, or a related field.
Navigational Expertise: Strong understanding of justice system processes, post-release support services, and working effectively in correctional settings.
Core Skills: Outstanding advocacy, communication, and interpersonal skills, along with proficiency in digital case management systems.
