Disability Support Worker

Location: CLOVERDALE, WA

Job Type: Permanent position | Full time position

• Support clients to participate in age appropriate actives and tasks
• Support client to access daily activities and outings in the community
• Provide direct care assistance to each client in accordance with their service plan and as directed
• Work in accordance with the medication administration policy, work health and safety procedures and client right's policies including privacy and confidentiality
• Monitor the safety and well-being of the clients and report any concerns
• Provide 1:1 or small group training and support in response to individual client needs
• Ensure clients' safety and well-being at all times.
• Ensure completion of appropriate documentation
• Maintain accurate records and report any changes in clients' conditions.
• Work with clients, family/carers, relevant case Team Leader/CEOs and other professionals to support the client's individual goals.

Skills & Experience Mandatory:

• Minimum Certificate III in Individual Support, Disability, or Aged Care (or equivalent)
• Minimum 1 year experience disability or community support
• Valid

First Aid and CPR certification

• NDIS Worker Screening Check
• Completion of NDIS Orientation Module
• Manual Handling training
• Medication Administration Certificate
• Strong communication and interpersonal skills

Desirable:

• Internal Feeding (Enteral Feeding) Certificate
• Certificate in Complex Bowel Care
• Training in Dysphagia Management
• Supporting Effective Communication
• Supporting Safe and Enjoyable Meals
• Completion of “Quality, Safety & You” module
