About the role

The role of the Community Connections Case Manager supports adults under 65 who are homeless or at risk of homelessness in the Hume, Merri-bek region, including people sleeping rough with complex unmet needs.

This program works closely within a multi-faceted team based at Ozanam House in North Melbourne. The program provides short to medium term intensive case management support via an outreach model.

This position is focused on working with the client to address their housing needs as well as addressing support needs that may impact their ability to sustain accommodation. This is done through the development of a holistic, goal orientated case plan, creating linkages, supporting clients to access appropriate mainstream and specialist services particularly housing and health services.

Case managers will support the range of needs and issues affecting people who are homeless or at risk of homelessness, including mental health, alcohol and other drugs, disability, justice, and family violence whilst managing competing demands and tight deadlines in a high-volume, complex human services environment.

Case managers will, under minimal direction and guidance regarding work priorities, be required to undertake complex operational work, including consistent outreach to clients, as well as undertake planning and co-ordination of activities within the Community Connections Program.

About You

You are a compassionate and capable community services professional with experience supporting people experiencing homelessness, disadvantage and complex needs. You understand the importance of providing person-centred, holistic support and can confidently build trusting relationships while maintaining professional boundaries.

You are comfortable working independently and collaboratively in a fast-paced environment, using sound judgement to manage competing priorities, risk and challenging situations. You bring a strong commitment to social justice, cultural sensitivity and supporting people to achieve positive and sustainable outcomes.

Key Requirements

• Demonstrated experience working with people who are homeless or at risk of homelessness.
• Strong understanding of the housing service system, including the Opening Doors Framework and housing options.
• Ability to assess and manage risk, conduct assertive outreach, and build trusting relationships with clients.
• Knowledge of intersecting service systems (mental health, AOD, NDIS, justice, and family violence).
• Excellent communication, problem-solving, and stakeholder engagement skills.
• Proven ability to maintain accurate case notes, records, and compliance with program and legislative requirements.
• Relevant tertiary qualification in social work, community services, or related discipline (minimum 1–2 years’ experience desirable).
• A valid Victorian Driver’s Licence.
