Aged Care and Disability Coordinator

Summary:

The Aged Care and Disability Coordinator plays a vital role in enhancing the quality of life for individuals in the Ngukurr community by overseeing and coordinating aged care and disability services. This position is essential for ensuring that services are delivered effectively and in accordance with government regulations, thereby promoting the well-being and independence of clients. The coordinator will work closely with various stakeholders to develop and implement care plans that meet the unique needs of each individual.

Responsibilities:

• Coordinate and manage aged care and disability services within the community.
• Develop and implement individual care plans in collaboration with clients and their families.
• Ensure compliance with relevant legislation and organisational policies.
• Conduct assessments to identify the needs of clients and recommend appropriate services.
• Facilitate training and support for staff to enhance service delivery.
• Monitor and evaluate the effectiveness of care services and make necessary adjustments.

Qualifications:

• Relevant qualification in aged care, disability services, or a related field.
• Minimum of 2 years’ experience in a similar role within the public sector or community services.
• Strong understanding of aged care and disability legislation and best practices.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively with diverse stakeholders.
• Strong organisational and time management skills.
• Proficient in using computer software for documentation and reporting.
