About the role

The Team Leader provides day-to-day coordination and practical leadership for a client support team. The role works alongside clients and Wellbeing Mentors to ensure supports are safe, consistent, person-centred and focused on building capacity, choice and meaningful participation. The Team Leader models high-quality practice, supports team accountability and ensures concerns are identified and escalated early.

Key responsibilities

• Coordinate the daily operations of allocated support teams, including routines, household activities, client schedules and communication requirements.
• Keep the client's goals, preferences, rights and support plan at the centre of team decisions and day-to-day practice.
• Support Wellbeing Mentors to teach, demonstrate and practise skills that build client independence, confidence and participation.
• Assist the Care Manager with roster coordination so supports align with assessed needs, available funding, employment conditions and safe staffing requirements.
• Provide practical guidance, coaching, role modelling and task clarification to team members within the authority delegated by the Care Manager.
• Support induction and client-specific orientation of new team members and identify training or competency gaps.
• Facilitate or contribute to team meetings, handovers and client reviews as requested.
• Monitor the quality and timeliness of shift notes, medication records, incident reports, household records and other required documentation.
• Maintain client files, household information and approved records accurately and securely.
• Support safe medication practices only within documented authority, training and competency.

About you

• Experience supporting people with disability, complex support needs or psychosocial disability.
• Demonstrated ability to coordinate day-to-day work and positively influence a team.
• Person-centred, neuro-affirming and strengths-based approach.
• Clear written, verbal and digital communication skills, including accurate objective documentation.
• Sound judgement, organisation and ability to manage competing priorities.
• Ability to provide feedback respectfully, accept accountability and escalate concerns appropriately.
• Commitment to inclusion, human rights, dignity of risk and equitable participation.
• NDIS Worker Screening clearance, where required for the role.
• Working with Children (Blue Card) clearance, where required for the duties performed.
• Current First Aid and CPR certification.
