At Poppy’s Home Care, we believe great home care starts with really knowing the person you’re supporting – not simply following a care plan or ticking a box.
We’re a local, family-owned and nurse-led home care provider, and we’re growing quickly.
We’re looking for a caring, capable and highly organised Care Partner to join our team, supporting clients across Rockingham and surrounding areas.
This is an opportunity to join Poppy’s at an exciting stage of our growth, working alongside our Senior Care Partner, Clinical Nurse Manager and our fabulous Support Team in the office – all working together to provide the very best experience for our clients and Home Care Team Members.

First and foremost, you get to know your clients.
Not just their care plan, funding or services – the person.
What matters to them? What’s changing? What are they finding difficult? What would help them continue living their life, their way, at home?
You’ll take ownership of your own group of clients and work with them, their families, our Home Care Team Members and other health professionals to coordinate their care and find solutions that work for the individual.
That might mean arranging a referral, reviewing a budget, changing services, organising equipment, responding to something a team member has noticed, speaking with family or finding a completely different way to achieve what matters to the client.
No two clients are the same – and we don’t believe their support should be either.
We have processes and Support at Home requirements to follow, but we don’t want our Care Partners to simply say:
“That’s our process.”
We want them to ask:
“What is this person trying to achieve, and how can we help?”

Great Care Partnering is about connecting the dots.
A referral isn’t just something to record. You’ll understand why it’s needed, how quickly it needs to happen, who needs to be involved – and make sure it actually happens.
A concern raised by a Home Care Team Member isn’t just another case note. It could be telling you something important has changed for your client.
You’ll think about the client, their family, their Home Care Team Members, their budget, their goals, their risks and the other professionals involved in their care.
We’re looking for someone who notices, thinks, prioritises, acts and follows things through.
At Poppy’s, we don’t just record information – we do something with it.
This is really important.
Care Partnering is very relationship-focused, but it’s also a busy coordination and administration role.
You’ll regularly work across:
Client records and care plans
Emails, phone calls and follow-ups
Referrals and service coordination
Client budgets and expenditure
Case notes and documentation
Different computer systems
Multiple clients and competing priorities
You don’t need to know our systems before you start, but you do need to be confident with technology, comfortable with numbers and budgets, able to type and document efficiently and capable of learning new systems quickly.
This is a busy role. You’ll need to work at a good pace without losing attention to detail – or losing sight of the person behind the paperwork.
You don’t need to arrive knowing everything about Support at Home or the Poppy’s way of doing things.
You’ll receive a comprehensive induction, ongoing guidance and development from our Clinical Nurse Manager, Senior Care Partner and experienced team as you build your knowledge and confidence in the role.
There is a lot to learn, so what matters to us is that you’re someone who learns quickly, retains information, asks questions and can put what you’ve learned into practice.
We value experience, but we value curiosity and adaptability just as much.
If you’ve worked in aged care before, you’ll need to be open to learning new ways of doing things rather than simply relying on “that’s how we’ve always done it.”
We’re growing too, and we want someone who is excited about learning, developing and growing with Poppy’s.
You’ll need a minimum Certificate III in Individual Support (Ageing), or an equivalent relevant qualification, together with experience that demonstrates your ability to coordinate, organise and take responsibility for client outcomes.
You may currently be working in care coordination, case management, team leadership, community care, aged-care administration or a similar client-focused role. Enrolled Nurses and Registered Nurses with strong coordination, administration and technology skills are also encouraged to apply.
While experience providing direct care can be valuable, this is not a support worker position. We’re looking for someone who is ready and able to take responsibility for coordinating the bigger picture of a client’s care.
Your current job title matters less to us than the capability, attitude and way of thinking you can bring to Poppy’s.

This is a full-time position working five days per week, and applicants must be available to work full-time across the working week.
This is primarily an office-based position, with approximately 1–2 days each week spent visiting clients in their homes across Rockingham and surrounding areas.
We think those visits matter.
You can learn a lot from a care plan, but sitting with someone in their own home, listening to them and understanding how they live gives you the bigger picture.
The remainder of your week will predominantly be based in the office, working alongside the Poppy’s team and coordinating your clients’ care.
You’ll need a valid Australian Driver’s Licence and reliable vehicle. A car allowance is included as part of the role.
You’ll take ownership of your clients, but you won’t be doing it alone.
You’ll have our Senior Care Partner, Clinical Nurse Manager and Support Team in the office around you, with support across scheduling, administration, accounts and service coordination.
We believe great home care comes from people working together, sharing information and helping each other find solutions.
You’ll also work closely with our Home Care Team Members out in the community. What they see, hear and notice when supporting our clients is an important part of understanding the whole picture.
Your clients are your responsibility – but providing great care is a team effort.
