Care Partner Support at Home

Full-time | Anglesea, Victoria

Due to the continued growth of our organisation, Country to Coast Home Care Services is seeking an experienced Care Partner to join our Support at Home team in Anglesea.

This full-time position is suited to an experienced aged care professional who understands the Support at Home Program and is committed to providing responsive, person-centred care.

• Coordinate high-quality, person-centred services for Support at Home participants.

• Develop, review and monitor care and service plans.

• Support participants to understand their funding, service options and responsibilities.

• Monitor participant budgets and service utilisation.

• Identify and respond to changing needs, risks and safeguarding concerns.

• Coordinate services with care workers, nurses, allied health professionals and associated providers.

• Maintain accurate participant records and compliance documentation.

• Support hospital discharge and other care transitions.

• Respond promptly to participant and representative enquiries.

• Contribute to audits, quality reviews and continuous improvement activities.

• Ensure services align with the Aged Care Act, strengthened Quality Standards, Statement of Rights and Support at Home Program requirements.

• Demonstrated experience in aged care care coordination, case management or a comparable role.

• Practical knowledge of the Support at Home Program and contemporary aged care requirements.

• Experience managing participant budgets, care plans, risks and service delivery.

• Strong communication, organisation and problem-solving skills.

• Ability to manage competing priorities in a busy environment.

• High-level documentation and computer skills.

• Commitment to participant choice, dignity, independence and person-centred care.

• Current National Police Check.

• Current Victorian driver licence and access to a reliable vehicle.

Relevant qualifications in aged care, community services, nursing, allied health or a related discipline will be highly regarded.

You are an experienced, organised and compassionate aged care professional who can build trusted relationships with participants, families, staff and service providers. You are confident addressing risks, coordinating services, managing competing priorities and maintaining accurate records to meet regulatory requirements.
