Description
The Clinical Assessment Review Officer is responsible for assessing residents’ clinical care needs, initiating assessment reviews following changes in health status, and supporting the development of accurate, person-centred care plans that promote positive resident outcomes.
The position works in partnership with clinical leadership teams across Aeralife homes to ensure residents’ assessed needs are consistently and accurately reflected in clinical documentation, care plans and AN-ACC classifications.
Reporting to the Clinical Assessment Team Manager, this Hawthorn-based position requires traveling to Aeralife VIC homes.
What you will be responsible for· Collaborate with the Clinical Assessment Team Manager and each home’s leadership team to ensure residents’ care needs are accurately reflected in assessments and care plans.
· Maintain current knowledge of clinical assessment, care planning and AN-ACC requirements.
· Review clinical documentation to confirm that changes in residents’ care needs, progress notes and care plans support the applicable AN-ACC classification or reclassification.
· Identify and discuss discrepancies or inconsistencies in clinical documentation with the clinical team and provide guidance to support appropriate care outcomes.
· Communicate regularly with the relevant manager regarding changes in residents’ care needs and submit requests for AN-ACC reclassification when required.
· Identify facility-level education needs, communicate them to the Clinical Team and provide reports to the Regional Operations Manager.
· Support Registered Nurses to complete required documentation accurately and within appropriate timeframes.
· Where practicable, attend sites during AN-ACC assessor visits to support the facility.
What We OfferAbout You
· Current registration as an Enrolled Nurse or Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
· Experience in clinical care systems, documentation and reporting.
· Demonstrated clinical knowledge and the ability to identify and assess changes in residents’ care needs.
· Sound knowledge of assessment and person-centred care planning in residential aged care.
· Demonstrated ability to ensure assessments and care plans accurately reflect residents’ needs.
· Knowledge of and experience with the Australian National Aged Care Classification (AN-ACC).
· Proficiency in Microsoft Office applications.
· Ability to identify changes in care needs and submit requests for AN-ACC reviews.
· Current NDIS Worker Screening Check.
· Current driver’s licence.
