The Opportunity

Are you a highly organised Project Coordinator with a passion for improving aged care services and driving meaningful change? Join AccessHC at a pivotal time in aged care reform and play a key role in the implementation of the Australian Government's Support at Home (SAH) Program.

This exciting opportunity offers the chance to work across a progressive and values-driven community health organisation, partnering with leaders, clinicians and service teams to coordinate projects that improve client experiences, strengthen compliance, and enhance service delivery. About the Role

Reporting to the Service Impact and Development Manager, the Project Coordinator - Support at Home will coordinate and support the successful implementation of the Australian Government's Support at Home Program and broader aged care reforms across AccessHC.

You will work closely with internal and external stakeholders to drive project activities, support organisational readiness, facilitate service improvement initiatives, and ensure compliance with evolving legislative and funding requirements. This role is ideal for someone who thrives in a collaborative environment, enjoys analysing data and processes, and is passionate about improving outcomes for older Australians accessing community-based care.

What You Will Be Doing Key Responsibilities

• Coordinate the implementation of Support at Home reform projects, ensuring milestones, deliverables and timelines are achieved
• Lead project tracking, reporting and documentation activities, providing recommendations to support informed decision-making
• Build strong relationships with internal and external stakeholders to drive collaboration, engagement and successful project outcomes
• Facilitate workshops, meetings and continuous improvement initiatives to support organisational readiness and service excellence
• Monitor compliance requirements, service agreements, policies and procedures to ensure alignment with aged care reforms and regulatory obligations
• Analyse service data and performance trends to identify opportunities for quality improvement, innovation and operational efficiency
• Support the development of policies, guidance materials, tools and resources to strengthen service delivery and compliance
• Manage project documentation, risk registers and reporting systems to ensure accurate and timely information

What You Will Bring Key Selection Criteria

To be successful in this role, you will demonstrate:

• Tertiary qualifications in Project Management, Allied Health, Primary Care or a related discipline (desirable, but not essential)
• Proven experience building collaborative and productive relationships with diverse stakeholders to achieve shared outcomes
• Experience working within Aged Care services or supporting consumers navigating aged care systems and programs
• Demonstrated experience in project coordination, service improvement initiatives, quality improvement programs or organisational change projects
• Ability to interpret legislative, funding or regulatory changes and develop policies, procedures and operational guidance materials
• Experience in contract management, vendor management and client management/reporting systems such as Trak, Power BI, or the ability to quickly learn new systems

• National Police Check
• International Police Check (if lived or worked overseas within the past 10 years)
• Working with Children Check
• Evidence of right to work in Australia
• Current Victorian Driver's Licence
