The Opportunity
Are you a highly organised Project Coordinator with a passion for improving aged care services and driving meaningful change? Join AccessHC at a pivotal time in aged care reform and play a key role in the implementation of the Australian Government's Support at Home (SAH) Program.
This exciting opportunity offers the chance to work across a progressive and values-driven community health organisation, partnering with leaders, clinicians and service teams to coordinate projects that improve client experiences, strengthen compliance, and enhance service delivery. If you enjoy working in project management, aged care reform, service improvement and stakeholder engagement. This Max term fulltime role until 27th June 2027, offers the opportunity to make a lasting impact on the future of community-based aged care.
About the Role
Reporting to the Service Impact and Development Manager, the Project Coordinator - Support at Home will coordinate and support the successful implementation of the Australian Government's Support at Home Program and broader aged care reforms across AccessHC.
You will work closely with internal and external stakeholders to drive project activities, support organisational readiness, facilitate service improvement initiatives, and ensure compliance with evolving legislative and funding requirements. This role is ideal for someone who thrives in a collaborative environment, enjoys analysing data and processes, and is passionate about improving outcomes for older Australians accessing community-based care.
Through your work, you will contribute to strengthening sustainable, high-quality aged care services that support people to remain independent, connected and living well at home.
